Specialised Support Work

We provide individualised support in the home and community, focused on building independence, confidence, and meaningful connection.

This includes but is not limited to sports and exercise programs, short-term assistance stays (respite), capacity building in self-management and housing options.

We can assist with:

  • ‘On the ground’ support alongside Support Coordinators
  • Family and nominee support
  • Collaborative communication across care and allied health teams
  • Administrative supports including understanding and navigating your plan
  • Report writing support
  • Connection to services and supports
  • Housing transition planning
  • Crisis intervention, forward planning & on call supports
  • NDIS review preparation and planning, including assistance at meetings

Domestic & Family Violence (DFV) Support & Facilitation

We provide specialised support across disability, wellbeing, and complex circumstances — including  domestic and family violence (DFV).  This work recognises the overlap between disability, trauma, and system navigation, with a focus on supporting individuals to increase safety, stability, and long-term outcomes.

This service recognises that people with disability experience violence at a higher rate, yet remain under-represented in DFV services, and aims to reduce barriers to engagement and safety.

We can assist with:

  • Increasing understanding of the intersection of disability and DFV, including identifying and responding to risk
  • Supporting individuals to recognise DFV, disclose safely, and access appropriate pathways
  • Facilitating connections across DFV, disability, health, legal, and community services
  • Strengthening referral pathways and supporting responses that are accessible, inclusive, and responsive to individual needs

Social & Community Participation

We provide support to build meaningful community connections, explore opportunities, and engage in activities aligned to each individual’s goals and interests.

This includes accessing and trialling different services, activities, and pathways to support confidence, independence, and participation in everyday life.

We can assist with:

  • Active participation in sporting and recreational activities that are meaningful to the individual
  • Social activities that reduce barriers and isolation
  • Maintaining family and social relationships
  • Personalised support to explore and access meaningful employment opportunities

Health and Wellbeing Support

We offer one-on-one personal training and personalised programs to support gross and fine motor skill development, as well as improvements in health and fitness goals.

These services can be delivered at an individual’s place of choice or at NotAGym in Carrum Downs. Jo has been a group and personal trainer in this area for nearly 20 years, bringing a strong commitment to a healthy body and healthy mind.

We can assist with:

  • Personal training at home or in a gym setting
  • Solutions to improve health and fitness goals
  • Building a lifestyle that supports confidence, self-esteem and overall mental and physical wellbeing

Support Coordination

Having the right level of support is key to getting the most out of your personal NDIS plan. We provide personalised services that provide implementation and ongoing coordination (Level 2) - with a focus on building capability in understanding your NDIS goals.

We can assist with:

  • Understanding your plan and using your plan towards your NDIS goals.
  • Review preparation and planning
  • Build your or your nominees ability to possibly manage your own plan
  • Access community and mainstream services
  • Connect with NDIS supports

Daily Living Skills

Living as independently as possible is a goal for many of our participants. We can help you complete your everyday tasks with the assistance of tailored recommendations from occupational therapists; speech pathologists, psychologists, behavioural practitioners, exercise physiologists and other allied health therapists.

We can assist with:

  • Comprehensive support in developing independence at home and in the community based on those recommendations including daily living skills
  • Ensuring there is an ongoing collaborative approach with your therapists & care team to enhance your overall quality of your life
  • Providing respectful assistance within a home enviroment to both the participant and their informal supports
